Prepay for Scheduled Initial Assessment

Thank you for scheduling an Initial Assessment. Due to the exclusivity of this appointment (we cannot put anyone else in this time slot) and the cost of travel, we must ask that you prepay the $300 Initial Assessment fee prior to confirming your appointment. This is done by clicking on the button to the right and paying by PayPal or Major Credit Card.

This fee is 100% refundable provided you cancel with at least 24 hours notice. Also, there is no fee for rescheduling if done with at least 24 hours notice. If you reschedule with less than 24 hours notice, we require a $150 rescheduling fee. This fee is again prepaid.
